What to expect on your first visit

1. Please plan to arrive about 15 minutes early to complete your paperwork or click on the links below to print out the forms you will need to complete and bring with you.

  • Registration Sheet
  • Medical History
  • Consent to Treat

  For Medicare Patients, there will be additional paperwork to complete at your initial visit.

2. Please wear loose fitting clothes.

3. When you arrive you will be asked for your:

  • Prescription for therapy from your physician
  • Photo ID
  • Insurance Card or Worker’s Compensation information.

4. We will collect any applicable co-payment or co-insurance prior to your visit. Insurance plans can be confusing. Our billing department is available at any time to discuss your plan and answer any questions you may have.

5. Your therapist will conduct an initial evaluation and perform treatment based on your diagnosis and physician’s instructions. Some patients will also be fitted for and receive a custom splint or brace.

6. Your first visit typically lasts about 60 to 80 minutes. Subsequent visits are typically 40 to 60 minutes.
